We at Living Life Our Way Entertainment want to make sure we have the most accurate and updated information along with avoiding people changing their demographics too frequently. For this reason we will need a photo of you making the same pose as the generated picture below to ensure it is you. Then we will need a picture of an ID that has the name you are requesting and a picture of you. Nicknames are okay, as long as they closely resemble the full name on the ID (example: Nicolas & "Nick")

WE JUST NEED PROOF THAT THE PERSON REQUESTING THE CHANGE IS THE PERSON THEY SAY THEY ARE AND THE NAME THE ARE REQUESTING TO CHANGE IS THEIR LEGAL NAME (OR A CLOSE NICKNAME TO THEIR LEGAL NAME). AND FOR ADD SECURITY ALL UPLOADS WILL BE DELETED AFTER UPDATE IS APPROVED OR DENIED.
